ADDITIONAL TERMS AND CONDITIONS

  1. All offers afford the option to elect upon the original application an Annual Automatic Benefit Increase (ABI). Coverage up to an additional $25,000 Life Insurance and additional AD&D of $50,000 at the amount of $5,000 Life Insurance with $10,000 AD&D each year for a maximum of 5 years. Rates are based on your age and smoker status. If you decline the ABI in any year, you forfeit the future opportunity to add coverage.
  2. You may cover an unlimited number of your children¹ for $10,000 Life Insurance coverage from age 15 days to age 19, 23 if a full time student. Flat rate of $1.95 per month applies.
  3. You may elect to receive up to 80% of your Life Insurance coverage, to a maximum of $100,000 in the form of Accelerated Benefit² in the event you are diagnosed with a terminal condition and have 12 months or less to live.
  4. Life Insurance Reduction Schedule
      • At age 65 reduces by 50%
      • At age 70 reduces to $10,000
      • Terminates age 80
  5. Supplemental AD
      • 200% of the amount of your Life Insurance coverage
      • Reduction Schedule
      • At age 65, reduces by 50%
      • At age 70, reduces to $20,000
      • Terminates age 80
  6. Medical question required. If medical question is answered unfavorably, you do not qualify for this promotional offer however, you can obtain voluntary accidental death and dismemberment insurance. You must be performing your normal activities for coverage to become effective.

Exclusions:
We will not pay benefits under this section for any loss caused or contributed to by:

      • physical or mental illness or infirmity, or the diagnosis or treatment of such illness or infirmity;
      • infection, other than infection occurring in an external accidental wound or from food poisoning;
      • suicide or attempted suicide;
      • intentionally self-inflicted injury;
      • service in the armed forces of any country or international authority. However, service in reserve forces does not constitute service in the armed forces, unless in connection with such reserve service an individual is on active military duty as determined by the applicable military authority other than weekend or summer training. For purposes of this provision reserve forces are defined as reserve forces of any branch of the military of the United States or of any other country or international authority, including but not limited to the National Guard of the United States or the national guard of any other country;


      • any incident related to:
        • travel in an aircraft as a pilot, crew member, flight student or while acting in any capacity other than as a passenger;
        • travel in an aircraft for the purpose of p arachuting or otherwise exiting from such aircraft while it is in flight;
        • parachuting or otherwise exiting from an aircraft while such aircraft is in flight, except for self-preservation;
        • travel in an aircraft or device used:
        • for testing or experimental purposes;
        • by or for any military authority; or
        • for travel or designed for travel beyond the earth’s atmosphere;
      • committing or attempting to commit a felony;
      • the voluntary intake or use by any means of:
        • any drug, medication or sedative, unless it is:
        • taken or used as prescribed by a Physician; or
        • an “over the counter” drug, medication or sedative taken as directed;
        • alcohol in combination with any drug, medication, or sedative; or
        • poison, gas, or fumes; or
      • war, whether declared or undeclared; or act of war, insurrection, rebellion or active participation in a riot.
  • Exclusion for Intoxication
    We will not pay benefits under this section for any loss if the injured party is intoxicated at the time of the incident and is the operator of a vehicle or other device involved in the incident.
    Intoxicated means that the injured person’s blood alcohol level met or exceeded the level that creates a legal presumption of intoxication under the laws of the jurisdiction in which the incident occurred

Accelerated Benefit Option4: Elect to receive a percentage of your life insurance in the event that you become terminally ill and are diagnosed with no more than a specified amount of time to live.

Funeral Planning Assistance2: Honoring a loved one’s life. Work with compassionate counselors that assist with customizing funeral arrangements with personalized one-on-one service.

Total Control Account®5: Provides your loved ones with a safe and convenient way to manage life insurance proceeds.

Automatic Benefit Increases: May increase your initial benefit amount.

Dignity Memorial®6: Get access to the largest network of funeral homes and cemeteries to pre-plan with a counselor and receive discounts on funeral services.

Waiver of Premium: Life insurance can continue and no further premium payment may be required if you become disabled prior to age 60 and remain disabled for six continuous months.

  2. Grief Counseling and Funeral Assistance services are provided through an agreement with TELUS Health. TELUS Health is not an affiliate of MetLife, and the services TELUS Health provides are separate and apart from the insurance provided by MetLife. TELUS Health has a nationwide network of over 30,000 counselors. Counselors have master’s or doctoral degrees and are licensed professionals. The Grief Counseling program does not provide support for issues such as: domestic issues, parenting issues, or marital/relationship issues (other than a finalized divorce). For such issues, members should inquire with their human resources department about available company resources. This program is available to insureds, their dependents and beneficiaries who have received a serious medical diagnosis or suffered a loss. Events that may result in a loss are not covered under this program unless and until such loss has occurred. Services are not available in all jurisdictions and are subject to regulatory approval. Not available on all policy forms.
  3. Will Preparation and MetLife Estate Resolution Services are offered by Metlife Legal Plans, Inc., Cleveland, Ohio. In certain states, legal services benefits are provided through insurance coverage underwritten by Metropolitan General Insurance Company, Warwick, Rhode Island.  For New York sitused cases, the Will Preparation service is an expanded offering that includes office consultations and telephone advice for certain other legal matters beyond Will Preparation. Tax Planning and preparation of Living Trusts are not covered by the Will Preparation Service. Certain services are not covered by Estate Resolution Services, including matters in which there is a conflict of interest between the executor and any beneficiary or heir and the estate; any disputes with the group policyholder, MetLife and/or any of its affiliates; any disputes involving statutory benefits; will contests or litigation outside probate court; appeals; court costs, filing fees, recording fees, transcripts, witness fees, expenses to a third party, judgments or fines; and frivolous or unethical matters.

MetLife or the Issuing Insurance Company bears the investment experience of such assets and expects to earn income sufficient to pay interest to TCA Accountholders and to make a profit on the operation of the TCAs. Regardless of the investment experience of such assets, the effective annual rate on the Account will not be less than the rate guaranteed on the welcome guide. The TCA and other available settlement options are not bank products and are not insured by the FDIC or any other governmental agency. In addition, while the funds in your account are not insured by the FDIC, they are guaranteed by each state’s insurance guarantee association. The coverage limits vary by state. Services and discounts are provided through a member of the Dignity Memorial® Network, a brand name used to identify a network of licensed funeral, cremation and cemetery providers that are affiliates of Service Corporation International (together with its affiliates, “SCI”), 1929 Allen Parkway, Houston, Texas. The online planning site is provided by SCI Shared Resources, LLC. SCI is not affiliated with MetLife, and the services provided by Dignity Memorial members are separate and apart from the insurance provided by MetLife. Not available in some states. SCI offers planning services, expert assistance, and bereavement travel services to anyone regardless of affiliation with MetLife. Discounts through Dignity Memorial’s network of funeral providers have been pre-negotiated. Not available where prohibited by law. If the group policy is issued in an approved state, the discount is available for funeral services held in any state except KY and NY, or where there is no Dignity Memorial presence (AK, MT, ND, SD, and WY). For MI and TN, the funeral services discount is available for “At Need” services only.

  8. All applications for coverage are subject to review and approval by MetLife.  If you choose to apply for increased coverage, the increase may be subject to underwriting. MetLife will review your information and evaluate your request for coverage based upon your answers to the health questions, MetLife’s underwriting rules and other information you authorize us to review. In certain cases, MetLife may request additional information to evaluate your request for coverage.  Coverage will be effective in accordance with the applicable policy and certificate after approval by MetLife. Only applicants who reside in a US state, the District of Columbia, or Guam, Northern Mariana Islands, Puerto Rico or US Virgin Islands are allowed to complete their SOH form online (where available). Otherwise, applicants will be provided with a paper SOH form.  Individuals residing outside of the US or in certain US territories must be on US payroll and be approved by MetLife before being provided with an SOH form.

What is AD&D?

AD&D insurance can pay you a benefit amount if you have a covered accident that results in paralysis or the loss of a limb, speech, hearing or sight. If you suffer a fatal, covered accident, benefits will be payable to your beneficiary.

What does AD&D cover?

This extra protection can help provide financial security should a sudden, covered accident take your life or cause you serious harm. This coverage complements your life insurance coverage and can help protect you 24 hours a day, 365 days a year. (Please see your Plan Summary for details.) This protection may cover you for:

  • Paralysis
  • Fatal accident
  • Brain damage or coma
  • Loss of limb, speech, hearing or sight

What special features would be included in my plan?

This plan can pay additional benefits if you die as a result of an accidental injury while:

  • Seat belt(s) are in use
  • Air bag(s) are in use while wearing a seat belt
  • Traveling on a common carrier (e.g., commercial airline)
